The completed registration for your dog will include three (3) forms and their attachments. Use this handy form to check off each item.
 Please Note the Change in Address and Fax # when you send in your forms!
- FORM 1 of 3 - REGISTRATION FORM
- Attach a copy of the dog’s registration.
- FORM 2 of 3 - MEDICAL FORM
- Attach a copy of the veterinary diagnosis.
- FORM 3 of 3 - RECORDS RELEASE FORM
- Obtain signatures of registered owner(s).
- Obtain signature of veterinarian.
